<p>It begins as a period of uncontrollable shivering and chills, followed by a fever as high as 105F. Finally, a period of profuse sweating occurs, which helps the body temperature return to the normal. The patient is left exhausted but otherwise temporarily free of symptoms.<br />
However, plasmodia released from dying red cells and those continuing to multiply in the liver infect other red cells and cause repeated attacks, usually every two to three days. If untreated, attacks can continue intermittently for years and nay be fetal, although the body gradually builds up defenses, against the disease. While malaria is already life threatening, one form of the diseases caused by the parasites Plasmodium Falciparum, releases all of the parasites into the blood stream at once and produces a single severe attack that is sometimes fatal. Falciparum malaria may be complicated by kidney failure or disseminated intravascular coagulation (DIC).<br />
What causes IT?<br />
Malaria is caused by single-celled parasites (plasmodia), usually transmitted to their human hosts via the bites of an infected female anopheles mosquito. This disease is most common in tropical areas.<br />
Malaria may be spread during a blood transfusion or by sharing needles for intravenous drug use.<br />
Pregnant women with malaria can transmit the infection to their unborn children.<br />
Prevention<br />
Before departing for a tropical or subtropical area, see your doctor or, preferably, go to a travel clinic when you can obtain information on the types of malaria (including drug-resistant malaria) presently found in the area of your destination. Recommendations may include taking the antimalarial drug chloroquine before leaving and for at least four weeks after returning. In a growing number of areas, such as Southeast Asia, Colombia, and Africa, plasmodia are resistant to chloroquine. For trips to such places, other drugs, such as mefloquine or doxycycline are necessary.<br />
When in mosquito-infested areas, put mosquito netting over beds, use insect repellents, and install window screens. Wear long-sleeve-shirts and long pants of you go out during mosquito-prone hours. Stay indoors between dusk and dawn, the hours when mosquitoes feed.<br />
Diagnosis<br />
Patient history and physical examination are needed.<br />
Blood smears and examined for parasites.<br />
How to treat it<br />
Hospitalization is often required. The drug chloroquine is administered to kill parasites in the blood. To kill choloquine-resistant plasmodia, other drugs, such as quinine or quinidine, are used.<br />
For malaria other than that due to P. falciparum (which causes only one attack), an additional drug, primaquine, must be taken for two weeks. This kills parasites in the liver, thereby preventing recurrent attacks.<br />

Contact your doctor before traveling to areas where malaria is prevalent.<br />
See a doctor if malaria symptoms develop or recur.</p>

<p>having a “rice-water” consistency containing some mucus but no pus cells or blood. Low grade fever occurs in some patients. Vomiting may be projectile and is not accompanied by nausea with in 2-3hours, the massive loss of fluid and electrolytes results in life-threatening dehydration with marked weakness and collapse. Fluid therapy is not initiated, renal failure with uremia will occur and death rapidly ensues. The illness lasts 1-7 days and is shortened by appropriate antibiotic therapy.<br />
Lethargy, seizures, fever, hypoglycemia, Acute renal failure, pulmonary odema during rapid fluid correction and death are its main complications.<br />
<strong>Diagnosis:</strong><br />
Diagnosis of this infectious disease is largely clinical. In endemic areas, any patient with severe, watery diarrhea should be considered a possible case of cholera until confirmed by laboratory investigations.<br />
There may be marked acidosis, hypochloremia, and hypoglycemia on stool microscopy, the characteristic darting motility of vibrios can be recognized in freshwater preparations. Diagnosis is also confirmed by stool culture.<br />
<strong>Treatment:</strong><br />
The mainstay of treatment for cholera is fluid and electrolyte replacement. Oral rehydration (ORS) given is the treatment of choice (100 ml/kg/4 hours) vomiting is not a contra-indication to oral rehydration.<br />
ORS can be given as much a patient can take. If the child is not able to take oral fluids, has an ileus, or in shock, IV fluids (Dextrose Saline or Ringer Lactate) are given. Push 20 ml/kg of dextrose saline or ringer’s lactate IV in 10-15 minutes and repeat if shock is not corrected. All this fluid should have glucose to prevent hypoglycemia. Food should be started as soon as fluids deficits are replaced. It will minimize nutritional impact of the illness.<br />
Antibiotics are given to shorten the duration of illness.<br />
Chloramphenicol (75mg/kg/day in three divided doses for 3 days).<br />
Trimethoprim-sulfamethoxazole and ciprofloxacin are also given with doctor’s advice.<br />
<strong>Prevention:</strong><br />
Most important method of preventing life-threatening cholera in infants is prolonged Brest-feeding.<br />
Hand washing, safe food and water supply and proper handling of sewage are important preventive measures.<br />
In endemic areas, water and milk must be boiled, food protected from files, and sanitary precautions observed.<br />
All patients with cholera should be isolated.<br />
Cholera vaccine provides 50% efficacy upto 3-6 months after vaccination. It is given parenterally as a twice-dose.<br />
It is not recommended for children younger than 6 months of age.</p>

(iv) The curl-up will be finished by lowering shoulders to the floor. Breathe in as we do so. This is counted as one curl-up and partner will count curl-ups.<br />
(v) This test will be completed in two minutes. The pace should be same while doing each curl-up correctly.<br />
Doing 35-40 curl-up in two minutes is a sign of muscle- strength fitness for both boys and girls in fifth grade.<br />

<p> Closed-angle glaucoma is relatively uncommon and is characterized by rapid and extreme elevations of IOP, often causing acute symptoms such as sever eye pain and rapid blurring of vision.<br />
All types of glaucoma occur as a result of damage to the optic nerve which is caused by raised pressure in the eye.<br />
Causes: Glaucoma usually occurs when fluid in the eye builds up, causing higher pressure than the eye can withstand. The primary cause of open-angle glaucoma appears to be an excessive buildup of aqueous humor, increasing IOP. When IOP remains elevated or continues to raise, fibers in the optic nerve are compressed and destroyed, leading to a gradual loss of vision over a period of years. A family history of visual-field loss due to glaucoma increases the risk of optic nerve damage. The use of inhaled steroids or nasal sprays with steroids appears to increase the risk of open-angle glaucoma. Closed-angle glaucoma is caused by a sudden blockage near the iris that prevents aqueous humor from reaching the trabecular meshwork.<br />
Other causes may include trauma, genetic disorders and low blood flow to the optic nerve.<br />
Signs and Symptoms:<br />
Many people will not experience any symptoms and their sight may seem perfectly normal. This is why it is important to have regular eye tests to check for glaucoma, especially as you get older.<br />
Acute glaucoma, when the fluid in the eye becomes completely blocked, can be painful as an attack comes on very quickly. There may also be nausea, vomiting and headache. Sever glaucoma usually appears as a red and painful eye. Facial pain, headache, blurry vision, rainbow-colored holes around lights, nausea and vomiting may also be signs of closed-angle glaucoma.<br />
Treatment: Glaucoma is a chromic disorder that cannot be cured. Open-angle glaucoma can often be treated safely and effectively by medical or surgical measures, through lifelong therapy is almost always necessary. Medications are nearly always used first. However, initial surgery is becoming a more common option.<br />
Eye drops are the most frequent medical treatment for glaucoma because they have fewer overall side effects than oral medication. Eye drops are used to reduce eye pressure by either increasing the eye’s ability to drain or by decreasing the amount of fluid it produces. If you notice any side effects from any drops you use, contact your doctor. There are six different types of medications that are administered to the eye to treat glaucoma.<br />
Beta-blockers, Alpha-stimulators, Miotics and epinephrine are leading medicines for glaucoma.<br />
Carbonic anhydrase inhibitor pills are generally used when optic nerve damage continues or seems highly likely despite maximal optical treatment. These pills are usually used to decrease fluid production. Vitamin C-an antioxidant available in supplement form also abundent in citrus, berries, tomatoes and other fruits and vegetables. It may help reduce eye pressure and benefit people with glaucoma.<br />
Prevention: In most people, glaucoma cannot be prevented. However, regular screening may help to identify people either with early stages of the disease. Control your weight and blood pressure. Serious eye injuries can lead to glaucoma. Wear eye protection when you use power tools or play high-speed rackets ports on enclosed courts, or other wise risk being hit in the eye.</p>
